Conditioning therapy with fludarabine, busulfan, and antithymocyte globulin were initiated for 6 days and intravenous tacrolimus (Prograf, Astellas Pharma, Tokyo, Japan) was started 1 day before allogeneic bone marrow transplantation at a dose of 0.03 mg/kg over 24-hour continuous infusion.
